- Do you have high blood pressure (systolic blood pressure of 140 mmHg or higher or diastolic blood pressure of 90 mmHg or higher)?
- Do you have a body mass index (BMI) of 25 or higher?
- Have you been hospitalized for heart problems or high blood pressure in the last 90 days?
- Do you have heart failure with reduced ejection fraction (HFrEF)?
